Diagnóstico por imagen de dientes retenidos en pediatría: Impacto en decisiones de cirugía maxilofacial con mención a dientes supranumerarios

Resumen
El diagnóstico preciso de las anomalías de la erupción dentaria en pacientes pediátricos representa un desafío multidisciplinario crítico en la odontopediatría y la cirugía maxilofacial. La retención de piezas permanentes y la presencia de dientes supranumerarios, con especial énfasis en el mesiodens, originan severas complicaciones morfológicas, funcionales y estéticas, tales como la reabsorción radicular de dientes adyacentes, la formación de quistes dentígeros y la alteración de la longitud del arco. Esta investigación evalúa el impacto del diagnóstico por imagen comparando la radiografía convencional bidimensional frente a la tomografía computarizada de haz cónico (CBCT) en la toma de decisiones quirúrgicas maxilofaciales en la población infantil. Mediante una revisión sistemática bajo la declaración PRISMA 2020 en las bases de datos PubMed, SciELO, LILACS, MEDES y Trip Database, se analizaron estudios publicados entre 2021 y 2026. Los resultados demuestran que la CBCT modifica la conducta quirúrgica planificada en el 43.6% de los casos complejos, permitiendo una localización tridimensional exacta, la evaluación de la cortico-medular ósea y la preservación de estructuras vasculonerviosas. Se concluye que la radiología tridimensional optimiza la predictibilidad del tratamiento, minimiza la morbilidad quirúrgica y guía intervenciones interceptivas bajo un balance riguroso de radioprotección pediátrica.
